  Applications for the 2008-2009 school year will be accepted beginning January 3rd.

• Special Needs Form (print only if your child has a special need)

• 680 “Blue” Immunization Form (must be original and can be obtained from your pediatrician)
• 3040 “Gold” Health Record Form (must be original and can be obtained from your pediatrician)

Pre-Kindergarten Required Forms:

• CCA Discipline Policy
• State of Florida Dept of Children and Families
• Swim Central Water Safety Questionnaire
• Elementary School Reference Form (if child is in school at this time)

CCA Procedure to Re-Activate Your Application
(Wait-Listed or Previous Students only - not current students)

Please submit the following to have your application re-activated: 

• Complete the Re-Activation Application (see below to download proper forms)
• Submit a check in the amount of $35.00 made payable to CCA
• Copies of the most recent report cards and standardized test scores
• Current teacher-recommendation letters (see below to download proper forms)
• Middle and High School Student Questionnaire (see below to download proper forms)
• Submit current church references (see below to download proper forms)
