Athletic Forms
All athletic forms must be on file in the athletic department before an athlete is eligible to participate.  Forms may be dropped off at the front reception area or faxed to 954-200-6128.  E-mail:  wendyt@ccaeagles.org  if you would like to verify that your student's forms are current. 

Required forms for students participating on the Varsity, JV, MS and 6th grade teams: 

• CCA Release of Liability Form 
• FHSAA EL2 Preparticipation Physical Evaluation Form (Doctor must sign, stamp, and fill out page 2) For the 2010-2011 school year you must have the updated (3/10) EL2 on file
• FHSAA EL3 Consent and Release of Liability Form For the 2010-2011 school year there is an updated EL3 (6/10), this form must replace older versions on file
• CCA Release of Liability for Transportation Form
