All athletic fees
must be paid in full at the time of registration before an athlete is eligible to participate. Payment is made on-line when registering. E-mail:

Required forms for for Varsity, Junior Varsity, Middle School, and 6th Grade teams:

According to FHSAA guidelines, all athletic forms must be on file and current in the athletic department before an athlete is eligible to participate. Forms may be dropped off at the front reception area or faxed to the athletic department at 954-200-6128. E-mail: kimberlys@ccaeagles.org to verify that your child's forms are on file and up to date, forms are valid for 365 calendar days from the date of the current physical. Please note: the physical form that is required is NOT the Broward County Health form, the FHSAA EL2 form must be used, they will not accept a substitute form.
