Sweet summertime is here! We hope your child is having a wonderful time off spending time with family and friends, fueling their faith, and gearing up for the new school year.
Be sure to check out MyCCAH’s summer resources board to see if your child is responsible for completing any summer reading and/or assignments. Summer work is designed to prevent the “summer slide” and help your child continue to flex his/her mental muscles so their transition back to school is a smooth one!

Medical Forms
- Authorization to Dispense Medication Form (required annually in order for the CCAH Clinic to dispense any medication to your child).
- Athletic ClearanceThe Florida High School Athletic Association (FHSAA) requires that all student-athletes be cleared prior to participation of any kind. Before you log-in, please gather the following documents and prepare them for upload:
- A Completed EL2 - FHSAA Preparticipation Physical Evaluation signed by a medical professional
- Proof of Insurance
- Uninsured students may purchase a policy here
- Certificates of Completion for the following NFHS Courses
